Care Team Involved
- With a Medical Director (35+ years of experience) and ARNPs on hand, Certintell’s Care Managers are overseen as care coordination is provided to the patient in the comfort of their home while engaging clinic staff on any changes to the evolving care plan.
- As a patient-centered care plan, close communication involves the patient’s PCP and required health professionals to improve the overall health and wellbeing of the patient.
- Certintell Care Managers ship cellular-enabled RPM devices directly to patients and provides support on-device setup and ongoing use. Wifi is not required.
