Founded in 2014, Certintell is a Care Management company that offers patient-centered services for Medicare, Medicaid, and Dual-eligible patients in partnership with medical clinics, pharmacies and insurance providers. Certintell acts as a virtual care clinic by providing programs that improve access to care, reduce the cost burden of chronically ill patients, improve patient health outcomes and has the opportunities to increase relevant outcome-driven ratings (eg. HEDIS).

Certintell clinical team members work with cutting-edge technology that is shaping the future of health care and are trained in remote monitoring enrollment, set-up, and abnormal vitals response. They are the key to identifying remote patient monitoring patients and educating those who may need further patient care for their health conditions.

  • With a Medical Director (35+ years of experience) and ARNPs on hand, Certintell’s clinical staff are overseen as care coordination is provided to the patient in the comfort of their home while engaging clinic staff on any changes to the evolving care plan.
  • Certified Clinical Health Coaches ship cellular-enabled RPM devices directly to patients and provide ongoing care.
  • Clinical and Patient Support Assistants are critical in providing a patient-centered approach to remote care. This involves closely monitoring vitals for high-risk patient populations and ongoing customer service to ensure patients have the best experience on their health journey.
