Job Description | We are seeking a dedicated and skilled PRN Registered Nurse to join our virtual team in providing Remote Patient Monitoring (RPM) / Remote Therapeutics Monitoring (RTM) and chronic care management (CCM) to patients across the country. This position will focus on supporting patients managing chronic conditions through remote monitoring and virtual coaching. The role offers flexibility with a predominantly remote work environment and the opportunity to increase engagement over time. In this role, you will work on an on-call or as-needed basis, with the position currently at 25% FTE, with the potential to increase hours over time. This will always remain a contract position. – Represents the core values of the Company. – Monitor and assess patient vital signs, symptoms, and health indicators through Remote Patient Monitoring (RPM) and Remote Therapeutic Monitoring (RTM) tools. – Recognize abnormal readings and intervene according to established clinical protocols. – Conduct remote follow-ups with patients based on flagged data to gather further clinical information. – Identify patients requiring immediate clinical intervention based on monitored data. – Triage patient needs effectively, escalating to Nurse Practitioners or other healthcare providers when necessary. – Prioritize patient cases based on clinical urgency. – Provide personalized, one-on-one coaching to patients managing chronic conditions (e.g., type 2 diabetes, hypertension, cardiovascular disease) to help them adhere to treatment plans. – Educate patients on disease management, symptom recognition, medication adherence, and prevention strategies. – Help patients implement their personalized care plans, focusing on lifestyle modifications like nutrition, physical activity, and self-care. – Conduct comprehensive assessments of patients’ health status, lifestyle behaviors, and readiness for change. – Work closely with Nurse Practitioners and other team members to ensure comprehensive and coordinated care. – Participate in virtual meetings and team discussions to review patient progress and contribute to care planning. – Maintain accurate, timely, and detailed clinical documentation in the AthenaHealth EMR system. – Contribute to comprehensive care summaries and health reports for healthcare providers. – Ensure all documentation complies with healthcare regulations and organizational policies. – Proficient in using telehealth and RPM/RTM tools such as PatchRx, along with EHR systems like AthenaHealth. – Ability to troubleshoot technical issues related to remote health monitoring systems and guide patients through technology-related challenges. – In-depth knowledge of chronic disease management protocols, particularly for conditions such as diabetes, hypertension, and cardiovascular diseases. – Stay up-to-date on best practices and clinical guidelines for chronic care and remote health interventions. – Support patients in adhering to their treatment plans, monitoring their progress, and adjusting care plans as necessary. – Ensure compliance with HIPAA and other relevant healthcare regulations, particularly concerning remote care and virtual health interactions. – Adhere to organizational policies and procedures related to patient privacy, consent, and data security. – Ability to work on a PRN (as-needed) basis, responding to patient needs and workload demands in a flexible manner. – Comfortable adapting to changes in patient volume, technology, and workflow processes. – Demonstrate empathy and compassion in all patient interactions, ensuring patients feel supported and engaged in their care journey. – Provide culturally competent care, respect patient diversity, and adapt communication to meet individual patient needs. – Continuously leverage the latest advancements in telehealth, RTM/ RPM technologies, and virtual care tools to enhance patient outcomes. This role will contribute to innovative practices and support the adoption of new technologies and care models. – Maintain the highest standards of care by consistently adhering to clinical protocols and quality guidelines. Participate in ongoing process improvements to ensure efficient, patient-centered care. Assist in refining workflows, identifying opportunities to streamline care delivery, and contributing to positive health outcomes. – Engage in continuous professional development to stay current with industry best practices and emerging trends in virtual care and RTM/RPM. Encourage a culture of learning and quality improvement, sharing insights with the broader team to elevate care standards. |
Qualifications | – Active Registered Nursing License in good standing, based in the state of Iowa. – Must have graduated from an accredited nursing program – BSN preferred, but not required. – Minimum of 2 years of nursing experience, preferably in a clinical setting – virtual health setting is a plus. – Experience or training in managing chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases. – Proficiency in using electronic health records (EHR) systems, with experience in AthenaHealth being a plus. – Comfortable working in a remote/virtual care environment, with the flexibility to engage in occasional in-person visits or meetings if required. – Strong interpersonal communication skills, particularly in providing virtual coaching and education to patients. – Ability to work autonomously, exercise clinical judgment, and collaborate with a multidisciplinary team. – PRN position at 25% FTE with the potential to increase over time. This will always remain a contract position. |
