Letter sent to Iowa Providers serving Rural Iowa:

    Greetings,

The answer to improved population health might just be found in the boxes below. No need to go searching under rainbows; The Iowa Department of Public Health (IDPH) has given us the funds to supply the gold directly at your doorstep.

With funding from IDPH, Certintell is a Managed Services Organization looking to partner with providers like you to reduce COVID-19-related health disparities and address Social Determinants of Health (SDOH). Heart disease and hypertension can be two major contributors to COVID-19 comorbidities. Unfortunately, these conditions see higher prevalence in underserved and ethnic minority communities. With this funding, Certintell has been gifted with the amazing opportunity to supply individuals with pulse oximeters to instill the importance of self-monitoring for preventive care, which could lead to healthier lives and less complications with diseases and viruses that develop beyond an individual’s control.

Certintell is a telehealth and remote patient monitoring (RPM) company with 8+ years of experience furnishing accessible, low-cost services to marginalized communities. With a mission to close care gaps across the United States, we partner with community clinics to craft sustainable virtual care programs that promote patient-centered and provider-coordinated care.

What’s Next? In this box, find four pulse oximeters to initially test these equity-inspired initiatives among your patients. We encourage you to try the devices with patients who may be at a disadvantage now, but who have the potential to change. If you’re excited to further advance this new standard for public health in Iowa with us, reach out and we can help coordinate additional program efforts. Afterall, we are a virtual medical practice based in Iowa and have a fully-functioning patient-provider portal – with a suite of care managers – ready to help your patients adopt healthier habits. If you are interested after the IDPH grant concludes in May 2023, we provide connected RPM services at no-cost to your traditional Medicare patients needing support from the comfort of their home with all devices being cellular-enabled, which means there’s no need for internet connectivity.

For all inquiries, including those related to this revitalizing, grant-funded project, please contact us via https://certintell.com, or our Care Management Specialist, Chris Allen, at 573-768-4802. Request more educational material, email: Ops@Certintell.com

Special thanks to the IDPH and their Iowa Initiative to Address COVID-19 Health Disparities allowing Certintell to implement strategies to address social determinants of health needs and work with social service providers and related organizations (traditional and nontraditional) to reduce COVID19-related health disparities in disproportionately affected populations.
