About Diabetes Prevention Program (DPP)

Diabetes Prevention Program (DPP) works to make it easier for people with prediabetes to participate in affordable, high-quality lifestyle change programs to reduce their risk of type 2 diabetes and improve their overall health.

Care Team Involved

  • Lifestyle coach to lead the lifestyle change program sessions and support and encourage participants (Coming soon via Certintell)
  • Program coordinator to oversee daily operations of the lifestyle change program, support and guide lifestyle coaches, and ensure that the program meets quality performance outcomes

  • A year long program is required

  • Prediabetes screening and testing is required

  • Care Coordinator works with the patient to create a care plan, covering all physical, mental health, and social service needs.
