Health coaches utilizing telehealth solutions will play a significant role in Chronic Care Management (CCM) services in an effort to improve patient outcomes and reduce the $3.3 trillion in annual health care costs.¹
DES MOINES, Iowa, March 26, 2019 — Certintell Inc. and Telligen have entered into a multi-year agreement to provide virtual health coaching to underserved patients, typically those on Medicare, Medicaid, or those with access issues, and seen through the nationwide network of Community Health Centers (CHCs) and safety-net hospitals (SNHs). By combining their expertise, Certintell will make Telligen’s health coaches virtually available to FQHCs to engage patients to manage their chronic conditions better. Certintell’s advanced telehealth platform allows health coaches to virtually work with patients through HIPAA-compliant video, audio, pictures, and text. Patients can connect to their health coach through their computer or through Certintell’s native smartphone app for iOS and Android.

According to the Centers for Disease Control and Prevention (CDC), an estimated 117 million Americans have one or more chronic health condition with one in four adults having two or more chronic health conditions. CCM is a critical step to support primary care initiatives and contributing to better health and outcomes for individuals with chronic conditions.
